The Texas.gov home page is available using the URL www.texas.gov.
From there, select:
- Occupational Licenses
- Health Care
- Medical - Primary Care
- Then the correct PIT Application (listed in alphabetical order) - Initial, Rotator or Institution Change.

Initial Physician in Training - select only if this is your first application for a PIT permit in Texas
Rotator Physician in Training - select only if you are a visiting resident from a program in another state. Your permit will be issued only for the length of your rotation; however, you can reapply online for a different rotation later, if needed.
Institution Change Physician in Training - select only if you have, or have ever had, a PIT permit, but are now transferring to a new institution for a new residency program. You do not need this permit if you are changing departments within the same residency program. Your program just needs to inform TMB of the change.
