All licensed physicians in the state of Texas are required to professionally use the name under which they are licensed with the Texas Medical Board. Using any other name may confuse or mislead the public and may be considered by the Board to be unprofessional conduct as defined by the Texas Occupations Code Chapter 164.052(a)(5).
Any physician who wishes to change the professional name under which he/she practices must first satisfy certain formalities and documentation. Any physician who is found to be using a name, other than the one that the physician is licensed under, may be subject to disciplinary action for violation of the above referenced statute.

This Form has been adopted by the Texas Medical Board in accordance with the requirements of §164.052(c), Texas Occupations Code, enacted by Senate Bill 419 (2005), and is published in 22 Texas Administrative Code §165.6(f). The purpose of this form is to allow the physician to obtain the required consents for an abortion to be performed on an unemancipated minor.

Under the requirements of Senate Bill 1330 (2005), physicians responsible for the management of an office that provides ongoing medical care to elderly persons are required by TMB to offer, to the extent possible as determined by the physician, the opportunity to receive the pneumococcal and influenza vaccines to each elderly person who receives ongoing care at the office. If the physician decides it is not feasible to offer the vaccine, the physician must provide the person with information on other options. As of April 1, 2008, practitioners who prescribe to Medicaid patients will be required to use tamper-resistant prescription paper when writing a prescription for any drug for Medicaid recipients.
