Online verifications for license and permit information on physicians, physician assistants, acupuncturists, medical radiological technologists, non-certified radiologic technicians, respiratory care practitioners, medical physicists, and perfusionists licensed by the State of Texas are provided by our “Look up a License” system. (This includes links to view Board Orders, where applicable).
The verification contains identifying information previously verified by the TMB, as well as the current status of the license.
Year of Birth: Birth year of the licensee.
License Number: License number for the licensee.
Issuance Date: Date the license was originally issued.
Expiration Date: Current expiration date of the license.
Current Status: Status of the license (Active, Delinquent, Suspended, Cancelled, Retired, etc.) and the date the status was last updated.
Disciplinary Restrictions: Any disciplinary restrictions on the license. “None” indicates that there are no restrictions to report.
Non-Disciplinary Restrictions: Any non-disciplinary restrictions on the license. “None” indicates that there are no restrictions to report.
Specialties: This will be blank for the majority of license types. Only some license types are limited by specialty.
School of Graduation: At the time of licensure, TMB verified the graduation from the school listed here. If there is no school listed, that does not indicate that TMB did not verify training. This only indicates that the school is not part of the public verification requirement for that license type.
Current Board Action: This section of the online verification contains the most current Texas Medical Board actions against this licensee. (This may include any formal complaints filed by TMB, as well as petitions and/or responses related to licensure contested matters, at the State Office of Administrative Hearings.) “None” indicates that there are no actions to report.
To request a name change, submit the form below along with the appropriate additional documentation. The form must be submitted with an original signature in order to be processed.
You must provide us with a statement that gives an explanation of your request, a completed Name Change Request form and a notarized copy of the legal document(s) that granted your name change, i.e. marriage license, divorce decree etc.

For practitioners wanting self-verification documentation. Upon written request TMB will provide a state board verification to another state board regarding licensees. The verification includes: licensee's name, license or permit number, issue date, expiration date, disciplinary status, and our board seal will be affixed. There is no fee for state board verifications. Please allow at least 10 working days to process your verification. The verification will be mailed directly to the state board of your request. Please note that providing a state board contact email on your form will assist us in expediting your request.

The TMB has introduced a new “Online Electronic Verification System” that will allow selected licensees to use their MyTMB account to request an automatic pdf verification of their license, permit or certification be emailed to the address of their choice. The pdf verification includes: licensee's name, license, permit or certificate number, issue date, expiration date, disciplinary status, and an electronic version of our board seal will be affixed. The system will create and email the pdf verification automatically once the request is submitted.
- To access this system log into your MyTMB account, select the “License Verification” icon and follow the directions.
- At this time the “Online Electronic Verification System” to request a verification is available to the license types listed below. If your license type is not listed, please submit the hard copy Verification Request Form. As the new electronic request option becomes available to more license types, they will be added to the list provided below.
