    This section of the physician profile contains identifying information previously verified by the TMB, as well as the current status of the physician’s license.

     

    Year of Birth: Birth year of the licensee.

    License Number: License number for the licensee.

    Issuance Date: Date the license was originally issued.

    Expiration Date: Current expiration date of the license.

    Current Status: Status of the license (Active, Delinquent, Suspended, Cancelled, Retired, etc.) and the date the status was last updated.

    Disciplinary Restrictions: Any disciplinary restrictions on the license.

    Non-Disciplinary Restrictions: Any non-disciplinary restrictions on the license.

    Specialties: This will be blank for physicians. physician licensure is not limited by specialty unlike some license types.

    School of Graduation: At the time of licensure, TMB verified the physician’s graduation from the medical school listed here. 

    Electronic RX Waiver Expiration Date: Expiration date of the electronic prescribing waiver, if such a waiver has been requested and issued.

    Current Board Action

    This section of the physician profile contains the most current Texas Medical Board  actions against this physician. (This may include any formal complaints filed by TMB, as well as petitions and/or responses related to licensure contested matters, at the State Office of Administrative Hearings.) “None” indicates that there are no actions to report.

    Medical Malpractice Investigations

    Section 164.201 of the Act requires that: the board review information relating to a physician against whom three or more malpractice claims have been reported within a five-year period for possible action.  “None” or a blank, indicates that there are no actions to report.  If action resulted See Current Board Action.

    Self-Reported Information

    This section of the physician profile contains self-reported physician identifying information including gender, country or state of birth, and race.  Also displayed is the physician reported current primary practice and information on the number of years that the physician has been in practice in the US or Canada, and the number of years of practice in Texas.

    Specialty Board Certification

    This section of the physician profile contains any physician reported specialty certifications issued by a board that is a member of the American Board of Medical Specialties or the Bureau of Osteopathic Specialists.

    Specialties

    This section of the physician profile contains the physician reported primary and secondary areas of practice.

    Education

    This section of the physician profile contains the physician reported name, location and graduation date of all medical schools attended by the physician.

     

    Also displayed is the physician reported graduate medical education taken in the US or Canada, including the following: Program name, Location, begin and end date, Type of program (residency, fellowship, etc.) and Specialty of the program.

    Hospital Privileges

    This section of the physician profile contains physician reported hospital privileges in the State of Texas.

    Utilization Review

    This section of the physician profile contains the physician report of whether he/she provides utilization review.

    Patient Services

    This section of the physician profile contains physician reports of participation in Medicaid, accessibility and language translation services offered by the physician.

    Awards, Honors, Publications

    The physician may optionally report descriptions of up to five awards, honors or publications in this section of the physician profile.

    Malpractice Information

    Section 154.006(b)(16) of the Act requires that: a physician profile display a description of any medical malpractice lawsuit against the physician where the physician was found liable, a jury awarded monetary damages to the claimant, and the award has been determined to be final and not subject to further appeal.   

    This section of the physician profile displays any applicable descriptions.

    Criminal History

    Section 154.006(b)(9) and (10) of the Act requires that: a physician profile display a description of (1) "any conviction for an offense constituting a felony, a Class A or Class B misdemeanor, or a Class C misdemeanor involving moral turpitude" and (2) "any charges reported to the board to which the physician has pleaded no contest, for which the physician is the subject of deferred adjudication or pretrial diversion, or in which sufficient facts of guilt were found and the matter was continued by a court." 

    This section of the physician profile displays any applicable descriptions.

    Non-TMB Disciplinary Actions

    Section 154.006(b)(12) of the Act requires that: a physician profile display a description of any disciplinary action against the physician by a medical licensing board of another state.

    Physician Assistant Supervision

    Physicians are required to register with the TMB if the physician supervises or delegates prescriptive authority to Physician Assistants (PAs)s.  Current registered supervisions and delegations to PAs are displayed in this section of the physician profile.
