Registration/renewal through TMB will be completed on a different schedule than DSHS, with all permits expiring during specific set times of year (as opposed to expiring in the permit holder’s birth month). Moving forward medical physicist licensees will expire once a year, on May 31st.

As a prerequisite to the registration of a surgical assistant's license, 18 hours of continuing education are required every 12 months, with at least half being Category I.  That is 36 hours of CE every 24 months (24 month timeline is in relation to the biennial registration period, not the calendar year).

If the request is granted, it may be granted without conditions or subject to such conditions which the board determines are necessary to adequately protect the public.  Requests are reviewed on a case by case basis.

Below are some examples of conditions that may be required (alone or in combination) to return a retired license to an active status:

• limitation of the practice of the requestor to specified activities of medicine and/or exclusion of specified activities of medicine;
• passage of the Special Purpose Examination (SPEX);
• remedial education, including but not limited to a mini-residency, fellowship or other structured program;
• passage of the Medical Jurisprudence Examination; and/or
• such other remedial or restrictive conditions or requirements which, in the discretion of the board, are necessary to ensure protection of the public and minimal competency of the applicant to safely practice medicine.

If you are granted a waiver, it will be displayed on your public physician profile.  This alone will serve as proof of your waiver.  Generally, waiver requests are determined within 2-3 business days.  You are encouraged to check your profile daily upon submitting your waiver request. You may also choose to print a simple copy of the ‘Verified Information’ section of your profile, which includes your waiver status, by choosing the “Print Verification” option on your profile.

The TMB no longer prints or mails hard copy permits but, if online verification is not sufficient, you can obtain a copy by setting up a MyTMB account on our website. https://sso.tmb.state.tx.us/Login.aspx.  The permit will be available for downloading or printing from there. Please be aware that in order to utilize the MyTMB portal you will need a valid Social Security number. If you do not have a SSN, you will not be able to access the service.
